This Privacy Policy explains how we collect, use, disclose, store, and safeguard your data when you use our platform and services in the United States.

1. Data Collection

1.1 Identity: Full name, DOB, address, email, phone, SSN (when required for insurance/regulatory compliance).

1.2 PHI: Medical history, medications, allergies, vitals, labs, imaging, ICD-10 codes, treatment plans, clinical notes, prescriptions, immunizations per HIPAA.

1.3 Financial: Insurance details (carrier, member ID, group), tokenized card data, ACH info, HSA/FSA data, billing history.

1.4 Technical: IP, browser, OS, device, screen, GPS/IP geolocation, page views, session data, clickstream, referral source, search queries.

1.5 Communications: Chat transcripts, video recordings (with consent), voicemails, email content, SMS logs.

1.6 Tracking: Essential/functional/analytics/advertising cookies, pixel tags, fingerprinting for fraud prevention only.

3. Data Sharing

3.1 Treatment: PHI to actively treating licensed providers in our network.

These Terms govern your access to and use of all Get Well Fast Care. platforms, apps, telehealth services, and related offerings in the United States. Use constitutes binding agreement.

1. Eligibility & Accounts

1.1 Age: 18+ or state age of majority. Minors via parent/guardian with joint access.

1.2 Accuracy: Truthful, current, complete info required. False info → suspension or fraud referral.

1.3 Security: You safeguard credentials. No sharing. Report compromises to security@getwellfastcare.com.

1.4 Use: Lawful personal healthcare only. No commercial use, scraping, bots, reverse engineering.

1.5 Prohibited: Fabricating info, deceptive prescriptions, threats/abuse, malware, intrusion, system interference.

1.6 Geography: Available only where providers hold active state licenses. Platform shows eligible services by location.

2. Healthcare Disclaimers

2.1 Emergency: NOT for emergencies. Call 911 for life-threatening conditions.

2.2 Limits: Telehealth cannot replicate all in-person exams. Providers may require in-person follow-up.

2.3 No Guarantees: No specific outcome guarantees. Decisions made solely by licensed providers.

2.4 Independence: Providers are independent practitioners with full clinical autonomy.

3. Payments

3.1 Transparency: All fees displayed before booking. No hidden charges.

3.2 Insurance: Real-time verification when possible. Final coverage by carrier. Copays are estimates.

3.3 Responsibility: You owe all non-covered amounts. Due within 30 days.

3.4 Self-Pay: Discounted published rates. Payment at service unless pre-approved.

3.5 Methods: Visa, MC, Amex, Discover, HSA/FSA, Apple/Google/Samsung Pay, ACH. No cash for telehealth.

3.6 Plans: Affirm/CareCredit for $400+ under separate agreements.

3.7 Refunds: Full if we cancel pre-service. Partial if partially delivered. 10 days to original method.

3.8 Disputes: billing@getwellfastcare.com within 60 days. 30-day investigation. Disputed amounts held.

4. Cancellation

4.1 Standard: Free 24+ hours ahead via Platform/app/phone/email.

4.2 Late: Under 24 hours = $20 fee.

4.3 No-Show: Full visit fee charged.

4.4 Repeat: 3+ no-shows in 12 months → prepayment required.

4.5 Provider Cancel: Priority reschedule + full refund + $10 credit.

4.6 Emergency: Waived with documentation within 72 hours.

5. IP, Liability, General

5.1 IP: All content, software, designs are Get Well Fast Care property under US IP law.

5.2 License: Non-exclusive, non-transferable, revocable, for personal healthcare use.

5.3 Submissions: Feedback becomes Get Well Fast Care property without compensation.

5.4 Disclaimer: "AS IS"/"AS AVAILABLE." No implied warranties.

5.5 Consequential: No liability for indirect, incidental, consequential damages.

5.6 Cap: Aggregate ≤ greater of 12-month payments or $100.

5.7 Malpractice: Nothing limits malpractice rights. Providers carry required insurance.

5.8 Indemnification: You indemnify Get Well Fast Care, officers, directors, employees, agents, providers from claims.

5.9 Disputes: Try support@getwellfastcare.com 30 days. Then AAA Consumer Arbitration in USA. No class actions.

5.10 Law: State of California without conflict-of-law principles.

5.11 Severability: Unenforceable provisions modified minimally; rest intact.

5.12 Entire: These Terms + Privacy Policy = complete agreement.

5.13 Termination: Either party may terminate. Survival clauses remain.
